Powdering Subgingival Margins

There is no reason why porcelain bonding needs to be limited to supragingival preparations. However, if the margins cannot be seen they cannot be powdered. I find that exposing the margin of a subgingival restoration is quick, easy and predictable with electrosurgery. Electrosurgery is a billable procedure. Charging for this service helps compensate the clinician for the extra degree of difficulty the subgingival restoration presents. If the tissue is inflamed, there may be some bleeding following the use of electrosurgery. With the proper hemostatic technique bleeding can be easily controlled. I avoid using retraction string in porcelain bonding. I have found that once retraction string is placed, there is no good time to remove it. If it is removed prior to the bonding procedure, bleeding can be induced at a time when a totally dry field is necessary. Attempting to remove the string after the bonding procedure can be difficult since the string can be bonded to the tooth.

Hemostatic agents applied with the Dento-infuser tip from Ultradent (800)-552-5512 are extremely effective at controlling bleeding. This applicator tip has fibers on the end and allows the hemostatic agent to be "burnished" into the bleeding tissue. With this burnishing effect, the hemostatic agent is forced into the capillaries and bleeding quickly stops. (Note: There are times when the interproximal space is too narrow for the Dento-infuser tip to fit to reach the hemorrhaging tissue. In these cases merely express the liquid over the tissue and tamp it into the tissue with a cord packer.) For severely inflamed tissue where bleeding is difficult to control, Astringident X, also from Ultradent, is very effective. Astringident X is harsh to tissues and is used as a last resort. Attention Panavia users: The high acidity of certain hemostatic agents has been shown to interfere with the weaker acid of the ED Primer, with the consequence of weaker bond strengths. To eliminate this possible problem, use 30% H2O2, which is available from your pharmacist, as a hemostatic agent.
